This method requires that we enter all information manually. This method can be used for schools under 500 that want to get under way quickly by printing the form from the website and making their own copies. |

Optical Mark Response (OMR) |
|
This is the preferred method. Students use #2 pencils to fill in their responses. These sheets are read by our scanner and will speed up our return time. You need to order these sheets from us (free). You may do so by clicking HERE. |

Questionnaires |
A variety of questionnaires are available for you to use. They are divided into categories. Click on the form number you would like to preview. The forms are stored in Portable Document Format (PDF). You will need a copy of Adobe Acrobat Reader to read and print the forms. If you do not have Acrobat Reader you can download a copy from by clicking HERE . |
